Resident Assistant Duty Switch Form

From this page, you can request permission to switch duty with another member of your staff. Duty switches that are known ahead of time should be requested at least two days ahead of the change date.

Please note that you should NOT consider this switch confirmed until your supervisor emails you back with their approval. You can expect to hear back from your supervisor within two business days.

This form should be used for all duty switches. If this switch is an emergency switch (within 12 hours) you should contact the HDOD directly, via phone, as well.

Questions about this form should be directed to your supervisor. If your supervisor is out of town, you should still submit this form and then forward the confirmation to their supervisor.
